Everyone’s health and safety are our top priorities. We love capturing these moments with you and are happy to reschedule at no cost if anyone in your group responds yes to the below symptoms or exposure.
Please complete this 24 hours before your shoot. By sending this to you I am confirming I have had no symptoms and have had no known exposure in the last 14 days. I am currently fully vaccinated, and 2 weeks past my booster shot. Should anything change on my end before I shoot I will let you know immediately.
A couple of things have changed - so I just wanted to go over them real quickly.
- Masks - I am more than happy to wear one during our shoot, please indicate below. Regardless of location, I will be wearing a mask at all times for newborn shoots.
- As much as I can I will be keeping a safe distance from you (which I hate esp with kids) the good news is most photos are captured more than 6ft away anyways so this has little if any impact on our shoot.
Due to an increase in COVID and FLU cases regardless of your vaccination status please confirm that no one in the photos or assisting in the photos has had any of the following symptoms in the last 5 days or any known exposure:
Fever or chills
Cough
Shortness of breath or difficulty breathing
Fatigue
Muscle or body aches
Headache
New loss of taste or smell
Sore throat
Congestion or runny nose
Nausea or vomiting
Diarrhea
